REMINDER:  Students are not allowed to have any kind of over-the-counter or prescribed medicine with them at school.  Please make sure all medicine is delivered to the nurse in its proper/original container.

School Physical Forms

Your child will need a current physical every year in order to participate in most extracurriculur activities.  Please click on the appropriate physical history form in addition to the Insurance Verification and Concussion  forms below; then print and fill out your child's information for their physical evaluation.  The school system holds an annual free physical event each year in the spring.   You will submit these forms to your school prior to the day of physcials.

National Handwashing Awareness Month – The best health habit that you can practice daily
to maintain good health is handwashing. To learn more about handwashing go to:
As we enter this Christmas season we need to remember the best gift that we can give to our
family and friends is to take care of our health and give Christ first place in our heart and life.
To be healthy means that the physical, mental, emotional and spiritual parts of you are in
balance and you are taking care of each area completely. When we put priority on the spiritual
area that always helps to keep the physical, mental and emotional areas healthy!
To learn more about staying healthy: Read Romans 12: 1-2 and Proverbs 4:23.
